Section 1What is this form for?

Article 11 of Law No. 6698 on the Protection of Personal Data grants a set of rights to everyone whose personal data is processed: to learn whether their data is being processed, to request information if it is, and to request its correction, deletion or destruction, among others.

To exercise these rights you need to apply to us. The application does not have to take any particular form — but under the Communiqué on the Procedures and Principles of Application to the Data Controller there is minimum information an application must contain. The form below has been prepared so that you can apply without leaving any of it out.

Using the form is not compulsory. A free-form petition or email containing the same information is equally a valid application.

You can submit your application by any of the routes below. All are equally valid.

Writing "KVKK Application" in the email subject line helps your application reach the right people quickly.

Section 3What your application must contain

Under the Communiqué, your application must include:

1. Your name, surname and, if the application is in writing, your signature
2. For citizens of the Republic of Türkiye, your Turkish ID number; for foreign nationals, your nationality, passport number or identity number if you have one
3. Your residential or business address for service of notice
4. Your email address, telephone and fax number for notification, if any
5. The subject of your request — which of your rights you wish to exercise

If you have information and documents relating to the matter, attaching them to your application will allow your request to be assessed more quickly.

Applying on behalf of someone else

If you are applying on behalf of someone else, you must attach to your application a power of attorney containing the specific authority, or a document showing your authority as legal representative. Applications made without this document cannot be concluded, on grounds of personal data security.

Section 4Response time and fees

We conclude your application as soon as possible and in any event within thirty days, depending on the nature of the request. The period begins on the date the application reaches us.

We send our response in writing or electronically, according to the preference you state in the application. If your request is rejected, the reasons are stated clearly.

Fees

Applications are free of charge as a rule. Where the operation additionally entails a cost, the fee in the tariff set by the Personal Data Protection Board may be charged. Under that tariff:

• If the response exceeds ten pages, a processing fee may be charged for each page over that
• If the response is to be given on a recording medium such as a CD or flash drive, the cost of that medium may be charged

In such a case we inform you and obtain your approval before carrying out the operation. Where the error giving rise to the request originated with us, any fee charged is refunded.

Section 6Right to complain to the Board

If your application is rejected, if you find our response insufficient, or if you do not receive a response within the period, you may complain to the Personal Data Protection Board.

The period for complaint is:

• Thirty days from the date you learn of the response
• In any event sixty days from the date of application

Under the Law, in order to complain to the Board you must have applied to us first. Complaints made directly to the Board may not be examined, because that condition has not been met.
